What is 

Transition is a process which will be slightly different depending on your circumstances, your body, your experiences, who you are and how you relate to the social, private and public worlds you move in.
Transition often begins before you make any physical changes. Where transition finishes is up to you. Transition will not happen overnight or even within a year or two. It takes time. Transition can change your body and your appearance. You won’t be able to pick and chose these changes. Once you’re on testosterone, physical changes will happen according to your individual genetic makeup.
Transition starts with disclosure to your GP, to your loved ones, to your partner (if you have one), to your employer, but firstly, and most importantly, to yourself!
However you look at it, transition is a combination of social, medical and legal actions.
Transition can be divided up into three areas - social, medical and legal.
Social actionsSocial actions are all the things These are things to do with aspects of your appearance, the way  
you dress, your hairstyle, body language and behaviour. Some people find these are all they need to feel better and more comfortable in themselves.
Medical actionsMedical actions include all the things you need a doctor to help you with such as hormone treatment and surgery. These actions are permanent. Some men start looking at their medical options around the same time as they make social changes. For others, it can be months or years after.
When it comes to your medical options, you’ll need a doctor. You must be prepared to work with your doctors and actively engage with them. They will want to make sure of three things:

  1. you’re healthy,
  2. free of any mental illness that is making you think you need to transition (rare) and
  3. you can manage your treatment responsibly and safely.
Legal actionsLegal actions are all the things to do with the legal system. Legally changing your name, correcting your birth certificate and correcting your sex at Medicare are a few examples. The majority of these legal actions depend on permanent medical actions.
